I serviced this HealthRider H95T treadmill in Westbrook, Maine. The walk belt was loose and twisted. I cleaned and lubed the belt and deck, then tightened and adjusted the belt. The belt was fairly worn, so we ordered a new one.
UPDATE: The new belt came, however it was the wrong sized belt which was actually two inches too long. You can see that by the marked size of the new belt in the pics, as well as my measurement of the old belt.

You can also see that the rear roller adjustment bolt was turned back as far as it would go, with the new belt in place, which also caused the belt to hit the end cap. So, we re-ordered the correct belt which should be 20″ x 122″ and not 20″ x 124″.
I included some pics showing how to remove the trick motor cover, as well as the side rails.
